| Take a print of the relevant Service Request form, fill it up completely along with the signatures of all DEMAT Account holders. Submit to the nearest IDBI Capital branch, along with the necessary supporting documents. |
Your Correspondence Address is registered with us at the time of opening your
Demat Account. If it has changed, you need to communicate the new address to us
by sending us the
Change of Address Form.

Your Bank Account details are required to be registered with your Demat Account
as per NSDL requirements. Companies use this information for making direct
credits to the Bank Account or for printing the Bank Account details on cheques
and warrants sent to you. You are required to give Bank Account details at the
time of opening the account. They can be changed at any time, by sending us
the Change
of Bank Details Form.

You can designate an individual as a Nominee for each Demat Account. A nominee
is entitled to receive the holdings in case of death of the holder(s) of the
Demat Account. You can designate a Nominee at the time of opening the Demat
Account or any at any time later. You can also change or cancel a Nomination at
any time by sending us the Change
of Nomination Form.

| In the case of death of the sole
or any of the joint holders of a Demat Account, the existing Demat Account is
required to be closed and the securities transferred to the Demat Account of
the persons entitled to receive the same. This process is called
Transmission.. Transmission Form can be submitted by Surviving Holders/Nominee/Legal Heirs or Legal Representatives. |
